Health Care Coordinator The Health Care Coordinator is a Licensed
Clinical Social Worker or Licensed Clinical Professional Counselor
responsible for mental health assessments of homeless guests, as well
as the coordination of the on-site physical and mental health services.
Facilities Coordinator The Facilities Coordinator will work to insure
that Breakthrough facilities, building equipment, grounds, and vehicles
are well maintained and kept in good repair.
